Job Overview
• Full-time benefited position
• Position includes:
* computer-accessed vital sign/health status management;
* home visiting of patients to install and educate them on the use of telemonitoring equipment;
* providing education to patients regarding CHF, COPD and diabetes disease management.
Qualifications
At least one year of acute care or med/surg nursing experience. The ability to work independently within a supportive clinical care team. Basic IV and wound care proficiency. NH RN license required. Drivers license and auto liability insurance required.
