ASSESSMENT & OASIS RN/PHN
POSITION - 1. 0 F. T. E.
POSITION PURPOSE: Perform assessments for admissions
of adult clients to home health services approximately 50% of the time and
perform OASIS documentation in the office about 50% of the time.
QUALIFICATIONS:
Registered Nurse licensed in the State of Minnesota
with PHN certification.
Minimum of two years RN applicable experience,
preferably in home health.
Ability to assimilate data to create a plan
relevant to client condition.
Understanding of insurances and priority of their
utilization.
Experience documenting Medicare and OASIS preferred
but will train
Excellent communication and organizational skills.
Good computer skills and comfortable working in
databases
Must be able to provide insured vehicle for
transportation with a valid driver’s license. |
