Pediatric Case Manager Under the supervision of the Administrator the RN/LPN-Pediatric Case Manager acts as a patient care advocate. Coordinates, negotiates, procures services and resources for and manages the care of complex patients to facilitate quality, cost efficient patient outcomes. Must be RN/LPN with min. of 3 yrs. current nursing exp. Prefer Case Management background
Education and Experience: RN/LPN License with minimum 3 years experience. Pediatric experience preferred
Communication Skills: Communicate effectively in oral and written form with patients, family members, agency staff, other service agencies, and the general public. Must possess strong active listening skills. Equipment:
Ability to operate proficiently a personal computer, fax machine, printer, adding machine and copier.
Operate basic medical equipment (stethoscope, BP cuff…) Must have own transportation.
Cell phone required. (Provided)
Certifications and Clearances: Valid driver's license; proof of auto insurance. Background check, Physical, CPR, TB testing. Currently Licensed as RN/LPN in PA.
Knowledge: Experience and/or knowledge of disabilities and/or terminal illnesses, specific to pediatrics and how it affects the various aspects of an individual’s life. Possess strong knowledge of the principles of the cycle of life and death. ESSENTIAL JOB FUNCTIONS
Provide services in accordance to the C. A. R. E. mission statement. Responsible for adhering to all policy and procedures as outlined in the Employee Handbook and other departmental procedure manuals. Complete all documentation and plans according to individual goals and objectives.
Sustain permanent patient roster. Maintain contact with all ancillary providers on a scheduled basis that is appropriate to the patient’s particular status and needs. Perform inspections with nurses and families as a |
